UAB (University of Alabama and Birmingham) Pain Behavior Scale 
The UAB Pain Behavior Scale can be used to track the severity of chronic pain over time. This can help determine the level of pain control and to identify temporal associations that can influence management. The authors are from the University of Alabama at Birmingham

 

Uncomplicated Benign Prostatic Hyperplasia Score

Patient-completed measure relating to their symptoms of micturition, urine storage and sexual function. 15-item questionnaire divided into 2 parts: firstly a symptom score (0 = no symptom present, 3 = maximum severity), and secondly evaluating the influence of the symptom on the patient?s daily life (same rating). The 2 scores are multiplied to give a total score (voiding maximum score = 108, sexual function maximum score = 27)

 

Upper Extremity Functional Scale (UEFS)
Patient-completed questionnaire to measure impact of upper extremity disorders on function. It has 8 items, sleeping, writing, opening jars, picking up small objects, driving a car, opening a door, carrying a milk jug and washing dishes, scored on a 10 point scale

Upper Extremity performance Test for the Elderly (TEMPA)

Staff-completed index to measure daily arm activities in normal people aged 60 years and older. Consisting of 13 items of tasks and functional activities, scored for the time taken (seconds), functional independence and task analysis. The functional rating is measured on a 4 level negative scale where zero indicates successful task completion

Upper Limb Activity Monitor (ULAM)

The ULAM is based on ambulatory accelerometry. The generic ULAM outcome measure ULAM-%dyn represents the percentage of the 24-hour measurement period spent in dynamic mobility-related activities (i.e. walking, cycling, general non-cyclic movement). Subscales; ULAM-body describes the mean intensity of body activity (in g, ms-2), which can be regarded as a general measure for the intensity of everyday physical activity. The body-part specific ULAM outcome measures ULAM-isit and ULAM-istand represent the mean intensity of upper limb activity of the involved side, expressed in mean (scaled) motility values during the time the involved upper limb was active while the subjects were sitting and standing, respectively. ULAM-%sit and ULAM-%stand represent the percentage of upper limb activity of the involved side, expressed as the percentage of the time that the involved upper limb was active (i.e. exceeding a certain threshold in the motility value) while the subjects were sitting and standing, respectively. The lower the scores, the worse the functioning.
