Quality of Life After Stroke

Patient-completed questionnaire of 58 questions in 4 domains: working conditions; activities at home; family relationships including close personal relationships and sexual patterns; leisure time activities in and outside the home. Measured on a 3-point scale before and after the stroke

 

Quality of Life Score following Intensive Care

Quebec Back Pain Disability Scale
Quebec Back Pain Disability Scale (in Dutch)
Patient-completed 20-item questionnaire designed to assess level of functional disability in individuals with back pain using 6 domains of activity: bed rest; sitting/standing; ambulation; movement; bending/stooping; handling large/heavy objects. Items scored on a scale from 0 to 5 (score of 100 = maximum disability)
