Neck Disability Index
Patient-completed, condition-specific functional status questionnaire, based on the Oswestry low back pain questionnaire. 10 items including pain, personal care, lifting, reading, headaches, concentration, work, driving, sleeping and recreation. Each item is rated on a 6-point scale with scores varying between 0 and 50


Neck Pain and Disability Scale

Patient-completed visual analogue scale of 20 items measuring intensity of pain, interference with vocational, recreational. social and functional aspects of living and the presence and extent of associated emotional factors. Item scores range from 0-5 and the total score is a total of the item scores


Neck Pain Score of Jordan

Jordan et al described a method for measuring neck and associated radicular pain in patients with neck disorders. The study was done in Copenhagen Denmark

 

Neonatal Facial Coding System for Pain Evaluation in Newborn Infants

Grunau and Craig used facial actions to monitor pain in newborn infants. This can be used to monitor pain in premature and full-term infants. The authors are from British Columbia Children’s Hospital and the University of British Columbia in Vancouver

 

Neonatal Infant Pain Scale

The Neonatal Infant Pain Scale (NIPS) is a behavioral assessment tool for measurement of pain in preterm and full-term neonates. This can be used to monitor a neonate before during and after a painful procedure such as venipuncture. It was developed at the Children's Hospital of Eastern Ontario

 

Neonatal Morbidity Scale

The Neonatal Morbidity Scale was developed by Minde et al to evaluate the relationship between the severity of complications in premature infants and subsequent maternal-child interactions. The score is based on 20 of the most common significant problems seen at the Hospital for Sick Children in Toronto Canada

 

Neurodevelopmental Disability in Infants

A variety of neurologic injuries may occur to the fetus in utero or in the newborn infant after delivery. These may result from intraventricular hemorrhage hypoxia infection or other cause. The injuries may result in long term disability

 

Neurologic Rating Scale (NRS) / SCRIPPS / Scripps Neurological Rating Scale
Staff-completed global assessment scale of cognition, affect and mood. Linear scaled measurements between -1 and +4, with 0 as the baseline i.e. normal function. Specific weighting for vision, motor, sensory and cerebellar symptoms. Summed final score with a maximum of 100 points as an indication of normal neurological function

New York Heart Association (NYHA) Functional Classification in a Patient with Heart Disease

Nine Hole Peg Test

Staff-scored, patient-completed measure of manual dexterity. Patient places 9 pegs in 9 holes in a square board as quickly as possible. The result can be measured as number of seconds to place all pegs, the number of pegs placed in 50 seconds, or the number of seconds to place each peg

Northwick Park Index of Independence in ADL

The Northwick Park Index of Independence in ADL was developed to measure activities of daily life and the ability to caarry out these activities independently. It was developed at Northwick Park Hospital in Middlesex England

 

Northwick Park Neck Pain Questionnaire

The Northwick Park Neck Pain Questionnaire was developed to measure neck pain and the consequent patient disability. It is relatively simple to use and provides an objective measure for monitoring symptoms over time. It was developed at Northwick Park Hospital in Middlesex England

Nottingham Extended Activities of Daily Living Scale (EADL)

Patient or carer-completed measure of disability. Comprising 22 items grouped into 4 categories: mobility (including public transport); kitchen tasks; domestic tasks; leisure activities. Scoring is hierarchical with 4 possible responses, collapsed to 2 responses on analysis. It is designed for postal or interview administration

Nottingham Health Profile (NHP)
Patient-completed questionnaire to determine and quantify perceived health problems. 2 parts: 1) 38 weighted statements covering 6 areas (sleep, mobility, energy, pain, emotional reactions, social isolation); 2) 7 statements regarding specific aspects of daily life (employment, household chores, social life, relationships, sex life, hobbies, holidays). Completed by Yes/No tick boxes

 

NIHSS - NIH Stroke Scale

Nottingham Sensory Assessment

Staff-completed sensory assessment form designed to test tactile and kinaesthetic sensation and stereognosis in stroke patients. Tactile sensation tests awareness of a variety of stated tactile stimuli on listed parts of the body. Kinaesthetic sensation appraises appreciation of joint movement, direction and position sense. Stereognosis tested by identifying specific named objects placed unseen in the hand. All measured on a numerical scale 0,1,2 = absent, impaired, normal.

Numerical Pain Rating Scale (NRS)

Patient-completed 11-point scale rated between 0 (no pain) and 10 (pain as bad as it could be). It can be administered as a questionnaire or through direct interview. It can be used both as a clinical tool and for research purposes. 101-point scales are also used

 

Nutrition Score

The Nutrition Score is used to assess the subcutaneous fat deposits of the newborn infant
