Hamilton Rating Scale for Depression

The total Hamilton Depression (HAM-D) Rating Scale provides and indication of depression and, over time, provides a valuable guide to progress. Classification of symptoms which may be difficult to obtain can be scored as: 0 - absent: 1 - doubtful or trivial: 2 - present.
Classification of symptoms where more detail can be obtained can be expanded to: 0 - absent; 1 - mild; 2 - moderate; 3 - severe; 4 - incapacitating. In general the higher the total score the more severe the depression. HAM-D score level of depression 10 - 13 mild; 14-17 mild to moderate; >17 moderate to severe. Assessment is recommended at two weekly intervals.

Hannover Activities of Daily Living Questionnaire

Patient-completed questionnaire regarding functional disability. Comprises 12 questions scored on a scale of 0-2: 0 = either unable to do, or only with help; 1 = can do, but with some difficulty; 2 = can do without difficulty. Points are totalled (24 points maximum = 100%). In the referenced paper scores greater than 70% indicate low disability

 

Harris Score for Evaluating Arthritis of the Hip 
The Harris score was developed to assess patients with traumatic arthritis of the hip. It can be used to follow patients over time and to help plan management including the timing for surgical intervention. It can be used to assess patients before and after surgery to determine improvement

Headache Disability Index
Self-completed disability scale measuring the impact of headache on a patient's ability to function normally in daily life. It contains 25 items and measures change over time. The responses of 'yes' sometimes' or 'no' are scored 4, 2 and 0 respectively. It is simple to administer and interpret

Headache Scale (HS)

Patient-completed questionnaire regarding subjective measure of headache pain, modified from the McGill Pain Questionnaire. Consists of 30 adjectives most frequently used by headache sufferers. Correlation of the adjectives results in 7 cluster groupings, 5 sensory and 2 affective. Scored and manipulated to produce a final score on a 6-point scale. Full details of scoring not described

Headache-Specific Locus of Control Scale (HSLC)

Patient-completed questionnaire to measure the headache sufferer's beliefs about the factors that influence the onset and course of headache episodes and therefore the amount of control they have over their own headache disorder. Contains 48 items measured on a 5 point Likert-type scale

Staff-completed questionnaire aiming to measure the degree of functional disability
HIV Overview of Problem Situations Evaluation System (HOPES)
Cross-sectional, patient-completed questionnaire regarding perceptions of quality of life, adapted from the Cancer Rehabilitation Evaluation System (CARES). Consists of 163 items rated on a 5-point scale from ""not at all"" to ""very much"". A higher score indicates poorer quality of life and higher mood disturbance

 

Health Index of Grogogno and Woodgate
Grogono and Woodgate developed a simple health index for evaluating the state of a person's health. This can be used to determine a person state at a given moment and the need for treatment. It can also be monitored over time to determine a person's response to interventions. The authors were from the Royal Free Hospital in London and St. Andrew's Hospital in Essex United Kingdom

 

HELP Survival Model of Teno et al for Predicting the Survival of Older Hospitalized Patients

An older patient who has been admitted to the hospital may experience decreased survival after discharge depending on the severity of his or her underlying conditions. The HELP survival model can be used to estimate the patient's survival and can help identify patients who are at greatest risk. The authors are from 9 institutions in the USA with the lead author from Brown University

Henry Ford Headache Disability Inventory (HDI)

The Henry Ford Hospital Headache Disability Inventory (HDI) can be used to periodically evaluate a patient with headache. It can be used to determine the effectiveness of a management strategy over time.

 

Hip dysfunction and Osteoarthritis Outcome Score (HOOS)

HOOS consists of 5 subscales; Pain, other Symptoms, Function in daily
living (ADL), Function in sport and recreation (Sport/Rec) and hip related
Quality of life (QOL). The last week is taken into consideration when
answering the questions. Standardized answer options are given (5 Likert
boxes) and each question gets a score from 0 to 4. A normalized score (100
indicating no symptoms and 0 indicating extreme symptoms) is calculated for
each subscale. The result can be plotted as an outcome profile.


Hoehn and Yahr Scale

Staff-completed, hospital-based interviews and examinations. The measure attempts to name and define types of Parkinsonism in a clinically applicable manner. Three classifications of types of Parkinsonism are primary, secondary and indeterminate and are based on a combination of clinical data and presumed aetiology. Rated on an arbitrary scale (stages 1?5) based upon the level of clinical disability

HoNoS / Health of the nation outcome scales

Staff-completed functional measure. 18 items measured on a five-point scale. Designed to measure change in an individual over two or more points in time as a measurement of outcome for therapeutic intervention. Information for measure gathered from other workers treating the patient

Horizontal Arm Hold / Arm Endurance Test

Patient-completed clinical test recorded by staff, measuring the time the patient can hold both arms horizontally out to the side describing small circles

Hospital Anxiety and Depression Scale (HAD)

Patient-completed mood scale specifically limited to the 2 most common aspects of neurosis present in hospital practice: anxiety and depression. 7 items in 2 subscales, rated on a 5 point (0-4) scale

Houghton Scale


Patient-completed postal questionnaire to assess the daily functional use of a prosthesis by lower limb amputees. Four questions with specific answers are weighted to provide a summed final score thus assessing final outcome of rehabilitation. The maximum score is 12 with 9 or more indicating satisfactory prosthetic rehabilitation

Hunt & Hess Scale
