Calculated Osteoporosis Risc (SCORE)

The Simplified Calculated Osteoporosis Risk Estimation (SCORE) to Help Identify Women at Risk  can be used to determine if a woman should be evaluated for osteoporosis by bone densitometry. This is intended to make screening more cost-effective.

 

California Disability Rating System

The California Disability Rating System is used to assess the work capabilities of the injured worker and to make recommendations for their subsequent employment.

 

Canadian Cardiovascular Society Functional Classification of Angina

The Canadian Cardiovascular Society Classification of angina pectoris separates patients with anginal symptoms into groups based on the severity of their symptoms. The classification uses the extent of limitation on daily activities and the kind of physical activity which precipitates the anginal episode.

 

Canadian Neurological Scale
Staff-completed scale designed to detect change in neurological status in conscious patients. 2 sections: A, for alert/drowsy patients, assessing level of consciousness, orientation and speech (receptive and expressive.) If no comprehension deficit, section A1 assesses motor function, i.e. weakness. Section A2 assesses comprehension deficits and motor response, i.e. ability to maintain a position once placed or response to pain. B, for comatose patients: recommends use of the Glasgow Coma Scale. Total score plotted on a graph

Canadian Occupational Performance Measure (COPM)

Staff and patient-completed individualised generic measure to detect self-perceived changes in occupational/ADL performance problems over time. Areas of self-care, productivity and leisure are generated by the subject and are scored according to importance of activities, level of performance and satisfaction with performance, yielding both total performance and total satisfaction scores

Cancer Inventory of Problem Situations (CIPS)

Patient-completed questionnaire documenting the psycho-social and physical problems of cancer patients consisting of 131 statements divided into 21 broad categories. The patient rates each response on a 5 point rating scale

Cancer Rehabilitation Evaluation System - Short Form (CARES - SF)

Patient-completed cancer-specific instrument measuring multidimensional quality of life. Contains 59 items out of the original 139 of the CARES measure. Each problem rated on a 5 point scale, zero representing ""not at all"" (no problem) and 4 = ""very much"" (severe problem). There is one single score, the Global - CARES SF, and 5 higher order factors representing: physical, psychosocial, medical interaction, marital and sexual. Some items do not fall into these categories but contribute to the final score

Capabilities of Upper Extremity Instrument
Staff-conducted telephone interview 32-item questionnaire assessing difficulty experienced in performing specific actions with one or both arms and hands in individuals. Four sub-scales included arm function (bi-lateral), right hand function, left hand function and reaching down. Responses stated on a 7-point scale representing self-perceived difficulty in performing the action with 1 representing ""unable to perform"" and 7 representing ""can perform without difficulty""

Caregiver Strain Index
Patient and main carer-completed questionnaire post discharge after hip or heart surgery, to measure strain experienced by informal carers. Comprises 13 items addressing aspects of carer/patient relationships. Yes/no answers totalled to give final score ranging from 0?13. Scores of 7 considered a risk level of stress

 

The Cedars-Sinai Health-Related Quality of Life Instrument (CSHQ-RA)

The CSHQ-RA (33 items – long version) is a multidimensional, illness specific questionnaire to assess health related quality of life. It measures the last 4 weeks retrospectively in 5 subcategories; physical activities, emotional welfare, agility, mobility and sexual function.

Chedoke-McMaster Stroke Assessment

Staff-rated measure of impairment in 6 dimensions (each on a 7 point scale) and disability in 15 activities

Children's Hospital of Eastern Ontario Pain Scale (CHEOPS)

The CHEOPS (Children's Hospital of Eastern Ontario Pain Scale) is a behavioral scale for evaluating postoperative pain in young children. It can be used to monitor the effectiveness of interventions for reducing the pain and discomfort

 

Children's Coma Scale (Modified Glasgow Coma Scale Adelaide Coma Scale Paediatric Coma Scale)
One of the components of the Glasgow coma scale is the best verbal response which cannot be assessed in nonverbal small children. A modification of the original Glasgow coma scale was created for children too young to talk.

COHMC Coma Scale for Brain-Injured Children
The Children's Orthopedic Hospital and Medical Center Seattle Washington) coma scale can be used to assess children with a variety of central nervous system injuries. Since verbalization is not assessed it can be used with preverbal or previously intubated children.

 

Chronic Disability Index of Waddel and Main for Patients with Low Back Pain

Waddell and Main used a simple disability index to evaluate patients with low back pain. The authors are from the Western Infirmary in Glasgow Scotland. 

Chronic Pain Grade Questionnaire / Graded Chronic Pain Questionnaire

Patient-completed graded classification of the global severity of chronic pain conditions. Pain is graded 1- 4, rating from low intensity through to severely limiting

Chronic Respiratory Disease Questionnaire (CRQ)

Patient-completed questionnaire to determine the effect of treatment on quality of life. The patient is asked to focus on 5 activities and the administered 20 questions are scored to produce measures of dyspnea, fatigue and emotional response. When subsequently retested focusing on the same activities, score reflects the change: 1/2 point should reflect significance

 

Cincinatti Knee Rating  System

The Cincinnati Knee Rating System includes a functional assessment based on 6 abilities important for participation in sports. This can help evaluate change following surgery or other intervention. The authors are from the Cincinnati Sportsmedicine Center and Deaconess Hospital in Cincinnati Ohio

Classes of Respiratory Impairment

The impairment to a person caused by a respiratory disorder can be assessed from pulmonary function tests. Both the American Medical Association (AMA) and American Thoracic Society (ATS) have developed schemes for rating impairment to the whole person

 

Classification of Rosser and Kind for Illness States

Rosser and Kind developed a classification of illness states based on disability and distress. This can be used to monitor a patient's illness over time and to compare the level of illness between patients. The authors are from Charing Cross Hospital in London.

 

Clinical Assesment Objectives for Pain Management

Gottlieb et al identified factors associated with successful pain management by a patient in rehabilitation for low back pain. This can help identify how motivated the patient is to do what it takes to manage pain

 

Clinical Dementia Rating (CDR)

Morris developed the Clinical Dementia Rating (CDR) to determine the impairment associated with dementia. The author is from Washington University in St. Louis

 

Clinical Mobility Scale

A Clinical Mobility Scale can be used to assess a patient's degree of mobility over time

 

Clinical Pain Assessment

Pain can be difficult for the patient to cope with and the clinician to monitor. A rating system allows for semi-quantitation which can be used over time to assess the adequacy of treatment and the progression of a patient's condition

 

Clinical Outcome Variables Scale (COVS) / Physiotherapy Functional Assessment

Staff-completed functional measure to quantify one aspect of mobility defined as 'movement from one postural position to another, or from one location to another, within walking or wheeling distance'. 10 mobility tasks are quantified using a 7-point scale

 

Clinical Prediction Rule for Prolonged Nursing Home Residence following a Hip Fracture

Some patients who have had a hip fracture will require a prolonged recovery in a nursing home. Steiner et al developed a rule to identify patients at risk for a prolonged disability using clinical factors available around the time of injury. The authors are from the University of Colorado

 

Clinical Prediction Tool for Functional Decline in elderly Women using Modifiable Risk Factors

Sarkisian et al developed risk scores to predict functional decline in the elderly women living in the community based only on modifiable risk factors. The evaluation can be performed in a primary care setting using minimal equipment. Women identified as at risk can then have a directed intervention to reduce their risk. The authors are from the University of California (UCLA and UCSF) and are associated with the Study of Osteoporotic Fractures Research Group.

 

Clinical Symptom Score of the Japanese Orthopaedic Association

The Japanese Orthopaedic Association (JOA) has developed a clinical symptom score for a patient with a herniated lumbar disc. This can help determine the degree of improvement following surgical intervention. The association is headquartered in Tokyo.

Colombian Rating Scale for Parkinson
Disease specific, staff-completed clinical observation schedule for monitoring drug therapy to assess the long-term effects of large oral doses of levodopa (L-dopa). Aims to assess the overall severity of the symptoms of Parkinsonism, specifically tremor, rigidity, akinesia, posture, gait, speech and handwriting. A 5-point scale is used with a maximum of 76 points


Columbia University Disability Questionnaire for Essential Tremor (CADET)

The Columbia University Disability Questionnaire for Essential Tremor (CADET Columbia University Assessment of Disability in Essential Tremor) is a 36 item instrument that can be used to quickly evaluate a patient with essential tremor. The authors are from Columbia University in New York City.

 

Columbia University Performance Based Function Test for Essential Tremor

Louis et al developed a test of functional impairment in essential tremor based on performance of specific tasks. This is a rapidly performed test that can help in initial evaluation and to monitor the patient over time. The authors are from Columbia University and the New York State Psychiatric Institute.


Community Integration Questionnaire (CIQ)

Patient or carer-completed 15-item questionnaire of 3 scales (home integration, social integration scored on 6 items, and productive activity scored on 2 items). Scores from productive activity are weighted to allow direct addition to other scores. An overall points score is derived. CIQ scores can be sub-divided to give analysis of each aspect of community integration

Community Mobility Assessment Outcome Rating

Staff-completed, patient-centred, inter-disciplinary 2-part assessment of functional ability in the community. In Part 1, goals and expectations before and after a community outing are identified by interview. In Part 2, 40 physical, cognitive and mixed functional activities are observed and scored over 7 sessions. Three-point scale; 0 - 3 indicates not /partially/fully accomplished

Community Outcome Scale

Staff-completed ordinal scale measuring community accessibility. Four dimensions: mobility; occupation; social interaction; engagement. Scales for each dimension range from 0 - 6, where 0 = community fully accessible; 5 = community inaccessible/unresponsive; 6 = in residential care. Developed from semi-structured interviews with patients and families and concurrent assessment of cognitive and functional assessments. Severity of trauma estimated and coded from medical notes

Comprehensive Head Injury Outcome Assessment

Staff-completed telephone interview questionnaire combined with review of medical records. Current disabilities, physical impairment, mood and frustration levels and their impact on life and work are scored. Personal factors and problems are noted. Findings are compared to initial disability and speed of admission for rehabilitation

Copenhagen neck functional disability scale
Patient-completed disability scale, consisting of 15 questions relating to activity, pain levels, headache, ability to sleep, concentration, work and leisure activities and psychosocial factors. Scored on a 3 point scale and summed to give a maximum score of 30 points, by post or in the clinic

Coping Strategies Index (CSI) / Coping Strategies Questionnaire (CSQ)

Patient-completed questionnaire studying the link between cognitive measures of coping with low back pain and the physical severity of LBP. 42 questions grouped into 8 categories (6 cognitive coping strategies and 2 behavioural coping strategies) scored on a 7-point Likert scale, and 2 individual question categories

Craig Handicap Assessment and Reporting Technique (CHART)
Staff-completed questionnaire to measure handicap. Consists of 6 scales, totalling 27 items: physical independence, mobility, occupation, social integration, and economic self-sufficiency. A total score represents handicap. (NB. Economic self-sufficiency scored using US government poverty scales.)

 

CRIES Scale for Neonatal Postoperative Pain Assessment

The CRIES (Crying Requires oxygen Increased vital signs Expression Sleep) scale is a tool for measuring postoperative pain in the neonate. The scale may be taken over time to monitor the infant's recovery or response to interventions. The authors are from the University of Missouri in Columbia

 

Criteria for Disease Activity of Osteoarthritis

Lequesne proposed criteria for disease activity in osteoarthritis affecting the hip or knee joints. The list has not been validated but reflects suggestions in the literature

 

Croft Shoulder Disability Questionnaire

Croft et al. developed a questionnaire concerning activities of daily life being hampered by shoulder disability
